Explain what happened, who was involved, what was affected, and how you found out.

Section 3: Impact & Action

Explain the actual or possible impact. Include client impact, business impact, legal risk, financial risk, security risk, reputation risk, or workflow impact.

Describe anything already done, such as notifying a manager, saving screenshots, removing access, changing a password, contacting a client, deleting a post, holding payment, or stopping work.

Upload or link screenshots, emails, messages, invoices, contracts, ClickUp tasks, Google Drive files, social media posts, client complaints, access logs, or other evidence.
